Background and Aim:Mandibular cortical erosion has been associated with systemic osteoporosis, and osteoporosis has been implicated in periodontal disease and tooth loss, particularly in older adults. However, evidence regarding the association between the Mandibular Cortical Index (MCI) and the number of remaining teeth in the Iranian population is limited. This study aimed to evaluate the association between MCI and the number of remaining teeth in Iranian individuals aged 40 years and older. Materials and Methods: In this analytical cross-sectional study, 341 patients referred to the Dental School of Islamic Azad University, Tehran, during 2021–2022 were included. MCI was classified into three categories (C1, C2, and C3) based on the Klemetti classification. The number of remaining teeth was assessed on panoramic radiographs, excluding third molars and residual roots. Demographic characteristics and bone health–related factors were collected using a structured questionnaire. Associations were analyzed using correlation tests and linear regression models. Result: A significant inverse association was observed between MCI and the number of remaining teeth (p <0.01), with fewer teeth observed in individuals with more advanced MCI classes. Age, body mass index, cigarette smoking, early menopause, history of osteoporosis, and vitamin D supplementation status showed significant associations with MCI classification (p≤0.1). Younger age, non-smoking status, and better oral hygiene practices were significantly associated with a higher number of remaining teeth. Conclusion:A lower number of remaining teeth is associated with a shift of MCI toward Class III. Assessment of MCI on panoramic radiographs may serve as a useful adjunctive indicator for identifying individuals at increased risk of tooth loss.
