Background and Aim:The impact of Micro thread of implant neck on marginal bone loss is controversial. This study aimed to evaluate the amount of marginal bone loss in dental implants with and without micro threads. Materials and Methods: A list of all patients who had received dental implants was prepared and they were recalled. Then, the rate of marginal bone loss and patient characteristics were recorded in terms of age, gender, number of implants, implant position (maxillary or mandibular), and implant type (micro-thread and non-micro-thread). The rate of marginal bone loss was measured in digital periapical radiographs using AxioVision SE64 Rel. 4.9.1 software. The data were compared using statistical tests including ANOVA (0/05=α). ،T test ، Mann-u-Whitney ,Post hoc Results:the study was conducted on 327 implants with micro thread and 315 implants without micro threads. The rate of marginal bone loss in micro threaded implants (mm 52/0± 32/0) was significantly higher in comparison with non-micro threaded(mm 58/0± 21/0) implants. (01/0>P) The highest bone loss rate of adjacent implants, both in the posterior mandible (P<0.05) and in the posterior maxilla (P<0.01), was related to implants with micro threads and higher than implants without micro threads. The highest bone loss rate of adjacent implants in patients with simultaneous bone grafting, in the posterior mandible (P<0.05) and in the posterior maxilla (P<0.01) was related to implants with micro threads and higher than implants without micro threads. The highest rate of bone resorption in adjacent implants in patients without simultaneous bone grafting in the posterior mandible (P<0.05) and in the posterior maxilla (P<0.01) was related to implants with micro threads and was higher than implants without micro threads. Conclusion:Considering the limitations of the present study, it seems that implants without micro threads show a lower rate of bone loss, and this difference was more significant in the posterior regions compared to implants with micro threads. however,further research is recommended.
