The effect of gelatin sponge containing aloe vera in preventing dry socket after the extraction of two first and second molar teeth of the mandible of patients

Background and aim: Alveolar osteitis (dry socket) is a common and painful complication following tooth extraction. This study aimed to evaluate the efficacy of an aloe vera-loaded gelatin sponge in preventing the occurrence of dry socket.
Materials and methods: This study was conducted as a double-blind, randomized clinical trial with a split-mouth design on 50 patients indicated for the extraction of the first and second mandibular molars. A standard pre-medication protocol (amoxicillin and chlorhexidine) was administered to all participants. In each patient, the extraction sites were randomly assigned to either the intervention side (placement of an aloe vera-loaded gelatin sponge) or the control side (routine extraction). Patients were assessed for the incidence of dry socket (alveolar osteitis) on days 3 and 7 post-operatively. Data were analyzed using the McNemar test.
Results: Out of 50 sockets in each group, 2 cases of dry socket (4%) were observed in the control side, whereas no cases (0%) were observed in the intervention side. This difference was not statistically significant (P=0.48).
Conclusion: The use of an aloe vera-loaded gelatin sponge was associated with a reduction in the incidence of dry socket, although this difference did not reach statistical significance. Further studies with larger sample sizes are recommended to confirm the definitive efficacy of this intervention
