fa بررسی رابطه میزان کورتیزول بزاقی با پریودنتیت متوسط تا شدید جراحی- oral surgery مطالعه اصیل original article <p><font face="Times New Roman" size="3"> </font></p><h5 style="margin: 0cm 0cm 0pt text-align: justify line-height: 115% unicode-bidi: embed direction: rtl" dir="rtl"><b><span lang="AR-SA"><font size="1">خلاصه: </font></span></b></h5><p><font face="Times New Roman" size="1"> </font></p><h5 style="margin: 0cm 0cm 0pt text-align: justify unicode-bidi: embed direction: rtl" dir="rtl"><font size="1"><b><span lang="AR-SA">سابقه و هدف:</span></b><span lang="AR-SA"> </span><span lang="AR-SA">بیماریهای پریودنتال از شایع ترین بیماری های حفره دهان هستند. هدف از این تحقیق بررسی رابطه میزان کورتیزول بزاق با بیماری پریودنتیت متوسط تا شدید در مراجعه کنندگان به بخش تشخیص دانشکده دندانپزشکی دانشگاه آزاد اسلامی تهران در سال 1390 بود.</span></font></h5><p><font face="Times New Roman" size="1"> </font></p><h5 style="margin: 0cm 0cm 0pt text-align: justify unicode-bidi: embed direction: rtl" dir="rtl"><font size="1"><b><span lang="AR-SA">مواد و روش ها:</span></b><span lang="AR-SA"> </span><span lang="AR-SA">این مطالعه از نوع مورد- شاهدی<span><font face="B Nazanin"> </font></span>بود.30 بیمار مبتلا به پریودنتیت متوسط تا شدید به عنوان گروه مورد و 30 فرد سالم در گروه شاهد قرار گرفتند .این افراد به تست مقیاس بیمارستانی اضطراب و افسردگی (</span><span lang="X-NONE" dir="ltr"><font face="Times New Roman">HAD Scale</font></span><span lang="AR-SA">) پاسخ دادند، همچنین شاخص های پریودنتال از جمله کنترل پلاک (</span><span lang="X-NONE" dir="ltr"><font face="Times New Roman">PI</font></span><span lang="AR-SA">) عمق پاکت (</span><span lang="X-NONE" dir="ltr"><font face="Times New Roman">PPD</font></span><span lang="AR-SA">) و میزان ازدست رفتن چسبندگی کلینیکی (</span><span lang="X-NONE" dir="ltr"><font face="Times New Roman">CAL</font></span><span lang="AR-SA">) سنجیده شد. یک سی سی نمونه بزاق هر بیمار جمع آوری و میزان کورتیزول بزاق آن به روش </span><span lang="X-NONE" dir="ltr"><font face="Times New Roman">ELISA</font></span><span lang="AR-SA"> بررسی گردید. بیماران در هر گروه از لحاظ عوامل مداخله گر مشابه سازی شدند و جهت بررسی یافته ها از آزمون کای دو و </span><span lang="X-NONE" dir="ltr"><font face="Times New Roman">odd's Ratio</font></span><span lang="AR-SA"> استفاده شد. رابطه بین نتایج تست </span><span lang="X-NONE" dir="ltr"><font face="Times New Roman">HAD</font></span><span lang="AR-SA"> و میزان کورتیزول بزاقی با استفاده از ضریب همبستگی </span><span lang="X-NONE" dir="ltr"><font face="Times New Roman">Pearson</font></span><span lang="AR-SA"> بررسی شد.</span></font></h5><p><font face="Times New Roman" size="1"> </font></p><h5 style="margin: 0cm 0cm 0pt text-align: justify unicode-bidi: embed direction: rtl" dir="rtl"><font size="1"><b><span lang="AR-SA">یافته‌ها:</span></b><span lang="AR-SA"> </span><span lang="FA">میزان سن در گروه مورد 92/7 </span><span lang="FA"><font face="Times New Roman">±</font></span><span lang="FA"> 77/41 و در گروه شاهد 94/7 </span><span lang="FA"><font face="Times New Roman">±</font></span><span lang="FA"> 73/40 بود. میزان امتیاز آزمون </span><span lang="X-NONE" dir="ltr"><font face="Times New Roman">HAD</font></span><span lang="FA"> در گروه مورد 10/6</span><span lang="FA"><font face="Times New Roman">±</font></span><span lang="FA"> 07/14 و در گروه شاهد24/4 </span><span lang="FA"><font face="Times New Roman">±</font></span><span lang="FA"> 27/11 بود و ارتباطی بین وضعیت استرس- افسردگی با میزان کورتیزول مشاهد نشد. Thirty patients with moderate to severe periodontitis were selected as case group and 30 patients were selected as control group. They answered hospital anxiety and depression scale (HAD scale) stress questionnaire. Periodontal indices such as plaque index and pocket probing depth and clinical attachment loss were assessed. <span style="mso-spacerun: yes"><font face="Times New Roman" size="2"> </font></span>Each subject provided a saliva sample of 1ml<span style="mso-spacerun: yes"><font face="Times New Roman" size="2"> </font></span>and the cortisol level was assessed by ELISA method. Both groups were matched regarding confounding variables (age, sex and smoking). Data were analyzed using chi-squared test and using Odds Ratio. Cut off point of salivary cortisol level was determined as 5.7 ng/ml by ROC curve method. There were 17 patients (56.7%) in case group and 4 in control group (13.3%) who exhibited salivary cortisol level over 5.7 ng/ml (p<0.0005).
