fa بررسی فراوانی ملانوم بدخیم سر و گردن در دوره 11 ساله در یک جمعیت ایرانی The Epidemiology of oral,head and neck malignant melanoma in an Iranian population during 11 –year perio مواد دندانی Dental materiall مطالعه اصیل original article   <h4 dir="rtl" style="margin: 0cm 0cm 0pt text-align: justify line-height: normal unicode-bidi: embed direction: rtl -ms-text-justify: kashida text-kashida: 0%">خلاصه: </h4>   سابقه و هدف: ملانومایک نئوپلاسم بدخیم بامنشاء ملانوسیتیک وسومین سرطان شایع پوست میباشد. ملانوم مخاط دهان کمتر از 1 درصد همه ملانوماها را شامل میشود. اتیولوژی مشخصی برای ضایعات مخاطی دهان معین نشده است و ملانوم مخاطی نسبت به همتای جلدی خود بسیار مهاجم تر است. به دلیل افزایش درفراوانی ملانوم درسالهای اخیروهمچنین فقدان اطلاعات کافی درخصوص آن لذا بر آن شدیم تا میزان فراوانی ملانوم سروگردن را در یک دوره 11 ساله در جمعیت ایرانی بررسی کنیم.   مواد و روش ها: این مطالعه با بررسی اطلاعات موجوددر پروندههای پاتولوژی بیماران مراجعه کننده به 5 مرکز مهم ارجاع این ضایعات، شامل بخش پاتولوژی دانشکده دندانپزشکی شهیدبهشتی، بیمارستانهای شهدا، لقمان حکیم، طالقانی و انستیتوکانسرامام خمینی انجام شد. در این مطالعه متغیرهای جنس،سن، محل ضایعه،عود،متاستاز، نمای اولیه ضایعه و محل سکونت بیمار بررسی شدند. جهت تجزیه و تحلیل اطلاعات به دست آمده از آزمون آماری chi-square استفاده گردید.   یافته ها: ازمیان 191679 پرونده ،262 مورد یعنی 14/0 درصد با تشخیص ملانوم سروگردن گزارش گردید که میانگین سنی ابتلا 01/11±50 سال بود.40درصد افراد مبتلا زن و60 درصد مرد بودند. بیشترین نمای بالینی تغییر رنگ به میزان 9/46 درصد وبعد از آن زخم به میزان 6/18 درصد بود. در مجموع در 8/24 درصد موارد متاستاز دیده شد. همچنین شایعترین محل ایجاد ضایعه در گردن و گونه و شایعترین ناحیه ابتلا در داخل دهان،کام گزارش گردید.   نتیجه گیری: فراوانی ملانوم سر وگردن در جمعیت مورد مطالعه 4/27 درصد کل ملانوما بود که بیشترین محل وقوع آن گردن و کمترین چانه گزارش شد. شایعترین محل وقوع ملانوم دهانی، کام بوده و این ضایعه در مردان شایع تر از زنان بود.   Abstract:  <div>Background and Aim: Melanoma is a malignant neoplasm of melanocytic origin that arises from a benign melanocytic lesion or denovo from melanocytes within otherwise normal skin or mucosa. Oral mucosal melanoma is much less than %1of all melanomas. Mucosal melanoma is much more aggressive than its cutaneous counterpart. Due to a real increase in the frequency of melanoma in recent years, we investigate the prevalence of malignant melanoma in the mouth ,head and neck during a 11 years period in Iranian population. </div> <div>Materials and Methods: This study was done on the pathology records of patients admitted to the Department of Pathology, School of Dentistry Shahid Beheshti ,Lughman Hakim, Shohada,Taleghani hospitals and Institute cancer of Imam Khomeini. In this study the variables like gender, age, location of the lesion, recurrence and metastasis was evaluated. The chi-square test was used to analyze information obtained from the research. </div> <div>Results: Of the 191679 cases accessed during the 11-year period, 262(0.14%) were identified as oral& maxillofacial malignant melanomas. The average age of melanoma was 50+-11/01years and the incidence in men was higher than women. The most common clinical feature was discoloration (46.9%)and then was ulceration (18.6%).Of all cases 24.8% had metastases. The most common site was the neck and cheek, and the most common area in the mouth, was reported in palate. </div> <div>Conclusion: Frequency of head and neck melanoma was 0.14% of all studied cases. The most common and the least common sites were neck and chin, respectively.
