fa بررسی انواع شکستگی های فک وصورت در موتورسیکلت سواران دارا و فاقد کلاه ایمنی مراجعه کننده به بیمارستان شهید رجایی شیراز Evaluation of various types of maxillofacial fractures in motorcyclists with and without helmets reffered to Shahid Rajaei Hospital in Shiraz جراحی- oral surgery مطالعه اصیل original article سابقه و هدف: ساختارهای فک و صورت معمولا در تصادفات موتورسیکلت ،تحت تاثیر قرار می گیرند. بنابراین ، هدف از این مطالعه تاثیر استفاده از کلاه ایمنی برفراوانی و الگوی شکستگی های فک و صورت در موتورسواران  بود. مواد و روشها:  دراین مطالعه مقطعی-توصیفی تعداد 181 پرونده از بیمارانی که با شکستگی های فک و صورت در بیمارستان شهید رجایی شیراز طی سالهای 1394 تا 1399 بستری شده بودند، مورد بررسی قرارگرفت..متغیرهای دموگرافیک و کلینیکی شامل سن،جنس،نوع شکستگی بر اساس منفرد یا متعدد بودن و محل شکستگی ها در مندیبل وناحیه میانی صورت  برای هر بیمار ثبت شد.داده ها به وسیله تست chi-square آنالیزشدند.(05/0 P<) یافته ها: از 181 بیمار،77 درصد بیماران مردو 8/22 درصد بیماران زن بودند. اختلاف معنی دار آماری بین استفاده از کلاه ایمنی وجنس بیماران وگروههای سنی مشاهده شد. (12/0=P)  در موتورسواران دارای کلاه ایمنی برخلاف موتورسواران فاقد کلاه ایمنی،شکستکی در ناحیه میانی صورت،کمتر از فک پایین بود.فراوانی شکستگی منفردومتعدد به ترتیب 3/39 % و 7/60 %بود. بیشترین فراوانی شکستگی های ناحیه میانی صورت و مندیبل به ترتیب در لفورت دو وتنه مندیبل بود.کمترین فراوانی شکستگی ناحیه میانی صورت و مندیبل به ترتیب دراستخوان بینی و راموس بود .برخلاف مندیبل ارتباط معنی دار آماری بین محل شکستگی های میانی صورت با استفاده از کلاه ایمنی وجود داشت. (011/0=P)  نتیجهگیری:  استفاده از کلاه ایمنی  می تواند باعث کاهش شکستگی های فک وصورت به ویژه در ناحیه میانی صورت شود.   Abstract Background and Aims: maxillofacial structures are usually affected in motorcycle crashes .So the aim of this study was to evaluate helmet using on the frequency and patterns of  maxillofacial fractures. Materials and Methods:In this descriptive cross-sectional study ,records of 181 patients hospitalized for maxillofacial fractures in Shiraz Shahid Rajae Hospital during 2016-2020were evaluated. Demographic and clinical variables included age, gender, type of fractures based on single or multiple and the location of the fractures in the mandible and the middle area of the face were recorded for each patient. The data were analyzed by  chi-square test(P <0.05). Result: Out of 181 patients,%77.2 of patients were male and  %22.8 were female.  Significant difference between helmet using and gender and age groups were observed. P= 0.012 In motorcyclists with helmet in comparison to without helmet,fracture in middle face area was lower than mandible.Frequency of single and multiple fractures were %39.3 and %60.7 respectively. The highest frequency of middle face area and mandibular fractures were in lefort II and body of mandible respectively. The lowest frequency of middle face area and mandibular fractures were in nasal bone and ramus respectively .Against mandible, there was a significant relationship between helmet using and middle face area fractures.
