fa بررسی دقت رادیوگرافی دیجیتال تفریقی در تشخیص پوسیدگی های ثانویه در دندانهای مولر دائمی Accuracy of Digital Subtraction Radiography for the detection of secondary caries in permanent molars(in vitro study) مواد دندانی Dental materiall مطالعه اصیل original article <h5 style="TEXT-JUSTIFY: kashida TEXT-ALIGN: justify LINE-HEIGHT: 115% TEXT-KASHIDA: 0% MARGIN: 0cm 0cm 0pt unicode-bidi: embed DIRECTION: rtl" dir="rtl">خلاصه: </h5><h5 style="TEXT-JUSTIFY: kashida TEXT-ALIGN: justify TEXT-KASHIDA: 0% MARGIN: 0cm 0cm 0pt unicode-bidi: embed DIRECTION: rtl" dir="rtl">سابقه و هدف: بدلیل محدودیت های رادیوگرافی معمولی در تشخیص پوسیدگیهای ثانویه، این مطالعه با هدف تعیین قدرت رادیوگرافی Digital Subtraction درتشخیص پوسیدگی های ثانویه انجام شد.</h5><h5 style="TEXT-JUSTIFY: kashida TEXT-ALIGN: justify TEXT-KASHIDA: 0% MARGIN: 0cm 0cm 0pt unicode-bidi: embed DIRECTION: rtl" dir="rtl">مواد و روش ها: مطالعه با طراحی تشخیصی بر روی 40 دندان مولر دائمی سالم که در محلول فرمالین 10 درصد نگهداری شده بود انجام شد. 80 حفره کلاس دو استاندارد در سطوح مزیال و دیستال دندانها ایجاد و با آمالگام ترمیم شد. دندانها توسط پوتی در فیلم نگهدارxcp ثابت شدند و تصاویر دیجیتال اولیه از هر دندان تهیه و در حافظه دستگاه ذخیره گردید. سپس پوسیدگی ثانویه توسط فرز روند 1 به عمق 1 میلیمتر در دیواره لثه ایجاد شده، توسط موم قرمز ترمیم شد. از دندانها مطابق تابش اولیه تصویر دیجیتال گرفته و تصاویر در حافظه دستگاه ذخیره شد. بعد از بررسی تصاویر توسط متخصصین، نتایج آماری با استفاده از نرم افزار (16) spss بررسی شد و شاخص های پنج گانه تشخیصی محاسبه شد.</h5><h5 style="TEXT-JUSTIFY: kashida TEXT-ALIGN: justify TEXT-KASHIDA: 0% MARGIN: 0cm 0cm 0pt unicode-bidi: embed DIRECTION: rtl" dir="rtl">یافته‌ها: PPV (ارزش پیش بینی مثبت) و) NPV(ارزش اخباری منفی) حساسیت و ویژگی به ترتیب، در ناحیه مزیال 6/71%، 4/75%، 6/69% و 2/77% و در ناحیه دیستال 77% ،87% و 86% ، 45/79% محاسبه گردید. درناحیه مزیال میزان خطای تشخیص DSR (2/26%) نسبت به ناحیه دیستال (4/17%) بیشتر بود که این اختلاف از لحاظ آماری معنادار بود. (001/0>P) </h5><h5 style="TEXT-JUSTIFY: kashida TEXT-ALIGN: justify TEXT-KASHIDA: 0% MARGIN: 0cm 0cm 0pt unicode-bidi: embed DIRECTION: rtl" dir="rtl">نتیجه گیری: به نظر می رسد استفاده از رادیوگرافی Digital subtraction در تشخیص پوسیدگی های ثانویه به خصوص در سطح دیستال دندان مفید است.</h5> Abstract: Background and aim: Due to the limitations of conventional radiography in the diagnosis of secondary caries. The purpose of this study was to assess the efficacy of Digital Subtraction Radiography in detection of secondary caries. Materials and methods: This Diagnostic in-vitro study was conducted on 40 healthy permanent molars stored in 10% formalin solution. Eighty standard CL.II cavities were prepared on mesial and distal surfaces of sample teeth and were restored using amalgam restorations. Teeth were fixed in XCP Film holder with Putty and initial digital images were taken of each tooth and were stored in a memory device. Cavities for secondary caries were created in gingival walls of each tooth by a no.1 round bur to a depth of 1 mm. and were restored by red wax. The images are reviewed by experts and the results were analyzed of using statistical analysis software SPSS (16). Results: The positive predictive value(PPV) and negative predictive vale(NPV), sensitivity and specificity,in the mesial surface were 71.6%,75.4% ,69.6% and 77.2% respectively and 77% , 87%, 86%, 79.4% in the distal surface. The diagnostic error rate in the mesial surface(26.2%)was higher compared to the distal surface.(17.4%), The difference was is statistically significant.
